Automatic Checking Account Draft

You may wish to become a Caring Angel by having an automatic draft from your primary or other checking account. Information needed to set up the automatic draft includes:

• bank name, address, and phone number

• name of account and owner

• account number

• designation for use of funds

• memorial information if appropriate

• if contribution is to be credited to a civic club or group, the name of the club, a contact name, and address of the club
